Breast augmentation is a common procedure to enhance the appearance of the breast. One complication that we try our best to avoid in breast implant related procedure is capsular contracture. There are many reasons why a patient might get capsular contracture. One common philosophy is the “biofilm” mechanism where an indolent bacteria enters the breast implant pocket and creates inflammation. The presence of the bacteria is not so strong to produce a fulminant infection resulting in pus, but one that creates subclinical inflammation. To prevent this in our population we clean the skin with antiseptic prior to surgery and we try to mitigate infections and the entry of bacteria. We have our patients eat a high protein diet, wear a compression bra, and limit activity. The maneuvers prevent the formation of a seroma post-operatively. A seroma is collection of fluid in the body. Small amounts of fluid are normal while larger volumes of fluid can become infected. Controlling this fluid, helps prevent cellulitis and other local infections that can lead to capsular contracture in the long run. Incision choice may help prevent the formation of capsular contracture and selection of appropriate implant size for the body. Genetics plays an important role in the formation of capsular contracture as come populations are more prone.
